This policy applies to consumer health data collected by Promptiva Health, Inc. (“Promptiva,” “we”) through promptivahealth.com (the “Site”). Joining our waitlist may reveal interest in a medication-management product, so we treat that information as consumer health data.

• Waitlist: Email address, optional name, and the fact that you joined the waitlist for a medication-management device. Used to manage the waitlist and send updates.

We collect this only with your consent. We do not collect information about your conditions or medications through the Site.

Directly from you, through forms on the Site.

3. Sharing
 

We do not share or sell consumer health data to any third party or affiliate. Service providers process it on our behalf under contract and may not use it for their own purposes.

• Confirm whether we collect, share or sell your consumer health data.

• Access it, with a list of all third parties and affiliates we have shared it with (currently none) and a way to contact them.

• Withdraw your consent at any time.

• Delete it, including from archive and backup systems (backups are purged within six months).

5. How to make a request
 

Email info@promptivahealth.com. We respond within 45 days, and may extend by up to 45 more days with an explanation.
 

6. Appeals
 

If we deny your request, email info@promptivahealth.com with “Appeal” in the subject line. We will respond in writing within 45 days. If the appeal is denied, you may file a complaint with the Washington Attorney General at atg.wa.gov/file-complaint or with your state's Attorney General.
